Forms and DocumentsThe RehabGYM recognizes that these are hard economic times. We are dedicated to the outcome of your rehab but also understand that money can be a bit tight these days. Depending upon your insurance situation, we have a variety of recommendations for your course of treatment.
- If you are on insurance with a high deductible AND you think you might reach your deductible this year, we recommend a traditional course of one-on-one care, as this will yield the fastest results and you will end up spending the money sooner or later.
- If you are on insurance with a high deductible AND you don't think you'll reach your deductible this year, we recommend a combination of consults ($85 per hour) and membership options*. You can schedule a one-on-one appointment with a clinician whenever you feel you want to check-in on a more in-depth level (members are encouraged to stay in touch with their clinician between consult appointments as well) and purchase a membership to perform most of your rehab independently.
- If you are on insurance with a copay or coinsurance, we recommend a traditional course of one-on-one care initially, as it will cost less out-of-pocket for you to pay for your portion of the visit than an entire consult. As your rehab progresses, your clinician will encourage a more independent approach involving consults ($85 per hour) and membership options* when appropriate.
- If you are uninsured, we recommend a combination of consults ($85 per hour) and membership options*.
